Please use the forms below to print and sign. The medical history forms need to be signed by the Riders physician whether the rider is a child or adult.
Printing instructions with PC
Click "Download file" for each document that you need copies of. Most web browsers will open a copy of the document directly in the browser, from here you can click the printer icon at the top right or save the file to your computer for your records.
If you are not presented with the form after clicking "Download file", you may have to save the file to your computer and install a free PDF viewer application like Adobe Reader. If you have any problems printing please let us know or contact us at 724-991-2477 [email protected] and we can email you the forms directly.
